IIRC the first screen in the survey says words to that effect. I'll try and remember to screen shot it next time.
At the end where you can put in your email address to go in the draw for whatever prize there is for completing the survey it also says that your email address will only be used to enter you in the draw and not to identify you in relation to your survey responses. That implies that the survey is anonymous.
As I posted above, anonymous or not it does not alter the way I respond.
I also fully expect the club to exclude my responses from any survey analysis because I am clearly ungrateful.
