In the VFA, the highest accuracy I found occurred in a Division Two match played on 21 April 1985:
Oakleigh 26.7 (163) vs. Dandenong 24.8 (152), an accuracy of 76.92%
Perhaps this is also the best accuracy rate anywhere for a game where both teams scored over 150 points!
